AI-Powered Patient Data Interoperability: A Framework for Clinical Excellence, Cost Reduction, and Patient Safety

Abstract:

Healthcare systems worldwide face rising costs, an aging population, and the growing burden of chronic diseases. Despite the widespread adoption of certified electronic health record platforms, seamless data exchange between hospitals remains limited, leading to inefficiencies, higher mortality, and increased medication errors. Patients receiving care across multiple providers are particularly vulnerable to fragmented information, resulting in avoidable harm, redundant testing, and unnecessary hospital visits.

This presentation introduces an AI-powered interoperability framework built on the SAP Business Technology Platform, which integrates electronic health records, IoT medical devices, and diagnostic systems into a unified ecosystem. The architecture combines advanced analytics, real-time event alerts, and secure data flows to support clinicians with timely insights and proactive decision-making. Clinical use cases demonstrate transformative benefits, including earlier detection of critical conditions, improved maternal and chronic care management, and safer medication reconciliation during transitions of care.

Operational improvements are equally compelling, with significant reductions in clinical documentation burdens, faster treatment initiation, and measurable decreases in readmissions and emergency visits. Hospitals implementing this approach report millions in annual savings, alongside enhanced patient satisfaction and improved staff efficiency. At a system-wide level, the adoption of interoperable, AI-driven frameworks creates opportunities for cost reduction, population health management, and precision medicine initiatives.

This session will highlight technical strategies, governance models, and outcome-driven case studies, demonstrating how AI-driven interoperability transforms fragmented healthcare systems into proactive, patient-centered models. Attendees will gain practical insights into building scalable solutions that simultaneously advance clinical outcomes, operational efficiency, and patient experience.
